Paranoia was a big one.

Stalin was always paranoid that someone was trying to kill him

to dive back further into history, the Roman appointed leader of the Jews, Herod went from a great leader to a tyrant who had most of his family killed off out of paranoia. His three remaining sons weren't very good at ruling and the entire area eventually came under direct Roman rule, which led to numerous wars and revolts

It's terribly dangerous and simplistic to attribute Hitler's or anybody's elses policies to an alledged mental disorder.

well i know hes not a former world leader, shakespeare, but he had a huge affect on history. I heard on some news program that they found traces of marajuana in his pipe, can anyone confirm that? I remember also seeing an old oxford scholar veremently denying that he could have smoked the "wacky tobacy"

Saparmurat Niyazov of Turkmenistan is probably the most insane leader today. He named himself Turkmenbashi (father of all Turkmens), and named cities, streets, an asteroid, January, and basically everything not nailed down after himself. He banned wearing beards, golden teeth and opera. He also wrote the Ruhnama, a book about himself and the history of Turkmenistan. Everybody who reads the book three times will automatically go to heaven according to Turkmenbashi. He has razed a large part of the capital to the ground in order to build palaces and statues, most of which are of course dedicated to himself. Currently on his orders an ice palace is being built in the Turkmen desert.

He has also been nominated for the nobel peace prize by some of his ministers, because of his "contributions to world history".

He also outlawed Ballet, so there must be a trace of sanity left somewhere.

Hitler was a repressed homosexual and some argue that's the reason why he murdered homosexuals because he could not accept his own homosexuality - a textbook case of "reaction formation" if you are a Freudian.
 
Of course homosexuality itself is NOT a disorder. Trying to repress it, however, could lead to depression and other pathological behaviours (e.g. homophobia manifested in violence and aggression).
